Power of Attorney Forms

A power of attorney is a document where you authorize a trusted friend or relative (an agent) to help you. The agent may present the document to a bank, hospital, or some other party to show they have your permission to help with your affairs.

Every person should name an agent to help in case an emergency arises.

This app prompts you to name a primary agent and a secondary agent for both health care decisions and financial decisions.

In regard to financial decisions you will have the option to give your agent special powers and you will need to decide when the agent's powers become effective - immediately or upon incapacity.

Designating an agent takes no power away from you and can help avoid invasive court proceedings in the event you need help and have not named an agent. Your agent does not have power to take action that is against your wishes and your agent has a duties to act in your best interests at all times.
