Health Care Directive

A health care directive is a document where you can express your wishes for health care in the event you are dying without any chance of recovery.

A related but separate document is called a Physician's Order for Live Sustaining Treatment (POLST) which you will complete with your doctor, most often when you are scheduled for a surgery or have received a life threatening diagnosis.

In the past health care directives have also been called 'Living Wills.'
